The following BDAT membership form is in two parts:
Part 1 - You will need to complete the following form with all the necessary
information and submit it. This form will be held in pending.
Part 2 - You will need to download the supplied pdf file, and mail it to the address
supplied with all the necessary details asked for.

Once we have your mailed information and completed our necessary inquiries, we will submit your details to our database.
You will receive notification of your membership via email with an official membership document posted out to your supplied mail address.
